Long-term Impact of Micronutrient Supplementation in Infancy on Growth and Neuro-cognitive Development: A Follow-up Study in Rural Sindh, Pakistan

Background

Adolescence, defined as 10–19 years of age, is a critical developmental stage marked by rapid physical, cognitive, and psychosocial growth, shaping lifelong health and behaviour. Global estimates highlight that adolescents represent the largest population in human history, with 1.8 billion adolescents worldwide, most of whom live in low- and middle-income countries, where opportunities to reach optimal developmental potential remain limited.

In Pakistan, adolescents constitute nearly 40 million individuals, accounting for more than 20% of the country’s population, with almost one-third living below the poverty line. Improving adolescent health is recognized as a global priority, with the Sustainable Development Goals and the Global Strategy for Women’s, Children’s and Adolescents’ Health 2016–2030 emphasizing nutrition and mental health interventions as key areas to promote wellbeing.

Despite extensive research on short-term outcomes of nutrition interventions during pregnancy and early childhood, evidence on their long-term effects remains limited. Systematic reviews and meta-analyses suggest that while supplementation and fortification improve micronutrient status and some immediate health outcomes, their impact on growth and neurodevelopment is modest and heterogeneous.

Malnutrition in adolescence persists as a major challenge, with South Asia showing high rates of thinness and stunting, alongside rising obesity. In Pakistan, national surveys reveal widespread undernutrition, overweight, and micronutrient deficiencies among adolescents, with significant implications for health, productivity, and intergenerational wellbeing. Given adolescence as a second window of opportunity for growth and brain development, understanding the long-term impact of early nutrition and education interventions on neurodevelopmental and nutritional outcomes is essential.

This cohort follow-up study aims to evaluate the long-term effects of early micronutrient supplementation on growth and neurodevelopment in adolescence.

The Institute for Global Health and Development is responsible for implementing this project in Pakistan.

Objectives

The objectives of the study include:

  • This study primarily evaluates the long-term effects of early nutritional interventions among infants at 6–24 months of age on physical growth and neurodevelopmental outcomes at 10–15 years of age.
  • The secondary objective is to examine key social determinants of health, including socioeconomic status, child and parental education, and food insecurity, as mediators and moderators of these effects.
