Patient Care

The palliative care team has a daily outpatient clinic catering to patients with chronic medical illnesses. Patients and families can access our services through referral by their primary doctors and or any other member of the team caring for them. We welcome patients and their families with questions to visit our offices located at the Cancer Care Centre or to contact us directly. The team also assist in providing key services to patients admitted to the inpatient wards including:

  • Maintaining patient autonomy.

  • Pain and symptom management.

  • Education on role of advance directives.

  • Help with establishing goals of care.

  • Encouraging family meeting to foster discussion.

  • Emotional, spiritual and psychological support for patients and their families.

  • Bereavement counseling.

  • Assistance with ethical questions and concerns.

  • Co-ordination and Continuity of care upon discharge.
