Cardiothoracic Aneasthesiology Fellowship Programme
The training in Cardiothoracic Anaesthesia is concerned with special problems related to perioperative anaesthetic and intensive care management of cardiac and thoracic surgery patients.
Objective
Upon completion of fellowship, the cardiothoracic anaesthesia fellow should have acquired knowledge, skills, ability, confidence and ethics to function as an academic cardiothoracic anaesthesiologist and consultant.
Duration of training
Two years
Eligibility
- MBBS or equivalent degree recognised by PM&DC
- One year house job or internship
- FCPS Part I is mandatory. FCPS Part II is preferred
- Four years structured training in anaesthesia
Scope and content
Clinical training
Year 1
- Operation Room (OR) training (cardiothoracic anaesthesia – Nine months)
- Cardiac Intensive Care Unit (CICU) – six week
- Perfusion service – two week
- Cardiology (Cath Lab, ECHO) – one month
- Presentation in monthly mortality and morbidity meeting, audit meeting, case conference, journal club
Year 2
- Operation Room (OR) training (cardiothoracic anaesthesia – eight months)
- Cardiac Intensive Care Unit (CICU) – two months
- Anaesthesia for Paediatric congenital heart surgery – one month
- Intraoperative TEE – one month
- Presentation in monthly mortality and morbidity meeting, audit meeting, case conference, journal club
Knowledge of subject
- Sound knowledge of anatomy, physiology pathophysiology, pharmacology, medicine, intensive care medicine, and surgery as they apply to the practice of cardiothoracic anaesthesia
- In-depth knowledge of cardiovascular and respiratory anatomy and physiology
- In-depth knowledge of common cardiovascular and thoracic diseases (IHD, valvular heart diseases, aortic pathology and common congenital diseases, common thoracic diseases requiring surgery) and knowledge of uncommon heart diseases. This includes diagnosis, investigations and management
- Interpretation of preoperative tests such as haematology tests, exercise testing, angiography, echocardiography, pulmonary function testing, ABGs, etc.
- Preoperative assessment, risk evaluation, intraoperative and postoperative anaesthetic management of patients presenting for cardiac, aortic and thoracic surgery
- In-depth knowledge of mechanical support devices such as heart lung machine, IABP and other circulatory devices
- Certification of Advanced Cardiac Life Support
Skills
Fellow should be able to perform, manage and interpret following procedures with minimal assistance from consultants: arterial catheterisation, central venous catheterisation, pulmonary artery catheterisation, double lumen placement/other lung isolation techniques, thoracic epidural insertion, fibroptic bronchoscopy, TEE for myocardial ischemia and contractility and valvular lesion.
Research/Scholarship
Fellow will be encouraged to attend educational workshops and actively participate in other Continuing Medical Education programmes in the University.
- Fellow will actively participate in different audits and ongoing research activities
- Fellow will complete one dissertation topic during fellowship. One publication in a Pakistan Medical and Dental Council recognised journal
- One national presentation every year
Evaluation
Continuous assessment will be done by all faculty members and a form will be filled out every three month along with viva evaluation by two consultants. There will be a MCQ test every six month. Feedback (verbal and written) will be discussed by one of the faculty members every three month.
Six monthly written and oral assessment and feedback (verbal and written) will be given by Programme Director.
Positions available
One fellow will be recruited each year.
Administrative set-up
The Cardiothoracic Anaesthesiology group includes eight full-time and one part-time consultant. A fellowship committee comprising three members including one fellow will look after the affairs of the fellowship programme.
