Urogynaecology and Pelvic Reconstructive Surgery
Urogynaecology, a sub-specialty of Obstetrics and Gynaecology deals with diagnosis and treatment of women who have pelvic floor disorders. These may include
• Incontinence: Loss of urinary or bowel control
• Prolapse: descent of pelvic organs (uterus,vagina,bladder,urethra,rectum )
• Difficulty urinating or moving bowels
• Pelvic or bladder pain
• Sexual dysfunctions
• Overactive bladder: pressure and frequent need to void
Department of Obstetrics and Gynaecology introduced pelvic floor services with trained Urogynaecologist and Pelvic Physiotherapist in 2004.The department is pioneer in establishing pelvic floor services including Urodynamics, EMG, Pelvic Nerve Conduction studies and Surgery for Stress urinary Incontinence in Pakistan.
We have estimated the burden of disease in rural Pakistan as Urinary Incontinence 11.5%and Utero-vaginal Prolapse 12.5%.Considering the magnitude of problem and shortage of trained Urogynaecologist in Pakistan .A Fellowship training programme was developed in year 2009.This involves multidisciplinary approach and collaboration with department of Physiotherapy, Urology and Colorectal Surgery to offer our Fellows a complete range of clinical and surgical training.
This highly competitive programme is the only programme of its kind in Pakistan and requires two additional years of training after completion of an accredited residency in Obstetrics and Gynaecology .The goals of the fellowship include leading the field through research and innovation with new and improved surgical methods for treating benign gynaecologic conditions (e.g. Stress urinary incontinence, vault prolapse, nulliparous patient with prolapse .and non-descent vaginal hysterectomy) with a severe impact on women’s quality of life.
Our Urogynaecology services offer all the state of the art treatment options for a variety of inpatient and outpatient volumes. The programme is designed in a way that training aspects are given equal attention to research and education through provision of protected time .The fellow is required to participate in all mandatory teaching sessions.
The fellowship curriculum has been developed according to the need of our population and by the end of fellowship we expect the fellow to
• Know the basis of pelvic floor dysfunction including urinary and fecal incontinence.
• Show an improvement in the quality of care of women with pelvic floor disorders
• Develop clinical competence to diagnose, evaluate common urinary, fecal and pelvic floor disorders.
• To perform surgery for stress urinary Incontinence, Utero-Vaginal Prolapse
• To conduct and interpret Urodynamic Studies ,EMG
• Demonstrate knowledge, practice and community teaching of urinary/fecal incontinence, pelvic floor injuries and defects.
• Develop a close understanding and working relationship with other disciplines like colorectal surgeon, pelvic physiotherapist ,urodynamcist ,involved in field of urogynaecology
