Obstetrics and Gynaecology 

The Obstetrics and Gynaecology Residency Programme at AKUH, Karachi commenced on November 1, 1987.  At present, there are 34 postgraduate trainees in the programme.  An important criterion for selecting the candidates is the desire to do postgraduate training in obstetrics and gynaecology and his/her willingness to undertake a rigorous four-year programme.  The training is approved by the College of Physicians and Surgeons, Pakistan and the Royal College of Obstetricians and Gynaecologists, UK.  The general guidelines for the programme are set by the central PGME Committee. 

Objective

Doctors completing the residency programme in Obstetrics and Gynaecology will be able to develop skills, knowledge and attitudes which will enable them to deal with obstetrical and gynaecological problems at a specialist level.  Residents who complete the programme will satisfy the requirements for FCPS Part II and MRCOG Part II Examinations. 

Eligibility

  • MBBS or equivalent degree approved by the PM&DC
  • One year internship, with a minimum of three months exposure in obstetrics and gynaecology
  • FCPS Part I/MRCOG Part I in Obstetrics and Gynaecology 
    Selected candidates are given an initial six months contract on the completion of which a yearly contract is given.

Duration of training 

The programme is of four years duration, of which three and a half year is spent in Obstetrics/Gynaecology, and six months are spent in elective training.  The exposure in Obstetrics and Gynaecology is a mix of tertiatry care as well as secondary care level.  The resident s are expected to rotate in the four secondary care centers of Aga Khan Hospital for specific time,which gives them ample exposure of community based obstetric problems.

Scope and content 

Clinical training 

The resident is exposed to a busy Obstetrics and Gynaecology Department with modern facilities for monitoring labour and delivery.  The Department conducts  62 outpatient clinics per week along with 24-hour emergency service.  Gynaecological operating sessions are held daily.  A  maternal medicine  clinic is conducted to deal with medical disorders in pregnancy and perinatal problems.  Foeto-maternal medicine service including level 2 ultrasounds for high risk pregnancies, diagnostic procedures such as amniocentesis and chorionic villus sampling facilities are also provided. It also provides counseling clinics along with paediatrics for those patients who require joint counseling and decisions.A modern and well-equipped neonatal intensive care unit under the Paediatrics Department provides care for neonates.  A separate infertility clinic is also held.  The Hospital has facility for urodynamic investigation to assist in the management of female patients with urological problems.  Gynaecological oncology services are provided and patients are managed jointly with the oncological team in the Medicine Department.

The off campus rotations include rotations in Garden, Karimabad, Kharadar and Hyderabad centers of Aga Khan University Hospital.  These centers offer a rich exposure to diverse obstetric populations and their management in a secondary level setup.  These centers also provide exposure to health related issues at a community level.

Didactic content 

The resident is expected to take an active part in all academic exercises.  These include participation in seminars, journal clubs, tutorials and case presentations.  S/he is expected to keep up with current literature on the subject.  Residents are involved in monthly morbidity and mortality meetings for Obstetrics/Gynaecology and perinatal cases.  They are also expected to teach junior residents, interns, nursing staff and medical students.  Each resident is also expected to take active part in research projects in the Department.  The clinical experience has to be documented in a log-book. 

Evaluation 

Knowledge and problem-solving skills are evaluated by departmental oral and written examination.  Continuous evaluation of residents is conducted by their supervisors and verbal/written feedback provided at the end of each rotation on quarterly basis.  Residents also evaluate their supervisors and the clinical rotation.  They submit a summary of their performance during each rotation.  Promotions are based on the overall performance of residents during the year.  Residents are expected to achieve a satisfactory performance in all domains of learning and patient care for promotion. 

On-call duties 

Residents are expected to work daily between 8:00 am to 5:00 pm.  In addition, they are expected to stay on the premises for on-call duty every third or fourth night.

Each year two or three new residents are taken into the programme. 

Administrative set-up 

The Department comprises  Associate Professors, Assistant Professors, Senior Instructors, Instructors and honorary faculty members.  At the departmental level, the programme is run by the Residency Director and a Departmental Postgraduate Committee.  The Residency Director in the Department is Dr Nadeem Zuberi,  Associate Professor.  The Coordinator for the programme is Dr. Nida Najmi, Senior Instructor.
